I have dark under eye circles and tired looking eyes, what causes these deep circles?

Patients often complain of dark undereye circles or hollowness under the eyes. This is often caused by atrophy of the fat under the eyes (the so called “tear trough”). This fat atrophy occurs with aging, but can be seen in men and women as early as their 20’s. Patients with significant fat loss under the eyes often complain of tired looking eyes.

What are the treatment options available for dark under eye circles?

Dark circles that are the result of fat atrophy can be treated well with injection of a filler containing hyaluronic acid into the area under the eyes called the tear trough.  Filling the tear trough can restore a more youthful and rested appearance.  The 2 best fillers that contain hyaluronic acid are Juvederm Ultra and Restylane.  Hyaluronic acid is a natural component of the dermis. How long does the filler last after it is injected? What are the side effects from injecting filler into the tear trough for treating undereye circles?

Studies show that these injections last 6 months to 1 year. About 1/3 of patient will experience bruising after the injection that will last a few days. Avoiding alcohol and Asprin prior to the treatment can reduce the chance of bruising.

What if I just have dark circles under the eyes but no hollowness?

Some patients have true hyperpigmentation under the eyes. This can be caused by sun damage, or chronic rubbing. If this is the case, lightening creams that contain hydroquinone or kojic acid can be helpful to reverse the darkening of the skin. In addition, sometime a series of chemical peels can also benefit the patient.
